Cause: Fault in tank floor caused a leak in the bottom of the tank.

Notes: Water was pumped into the tank in an effort to keep the naphtha level in the tank above the leak. A vacuum truck and pumps were deloyed to recover spilled naphtha on the gound. Contaminated soil was excavated and disposed of off site. No samples taken by LDEQ investigator.

Cause: Release from a safety valve. Valve was set to 100 psi instead of 260 psi

Notes: Naphtha overflowed sump into containment area. Naphtha recovered and transferred to wastewater treatment plant.

Tank 313 crude oil storage tank
Cause: The internal floating roof in Tank 313 sank in the tank and stopped controlling emissions from the tank.

Notes: Crude oil from tank 313 was removed and is being cleaned, inspected, and repaired. No LDEQ report included. BRQ: Tank 313 had all its contents removed and was cleaned so that repairs can be made, after which the tank will be put back into crude oil storage service.
