A leak developed on the channel head of the heat exchanger while it was being taken out of service for maintenance.

Notes/Remedial Actions
An operator was working nearby when the leak occurred. The operator went through HF decontamination on the unit and then complained of eye discomfort. The operator was initailly treated onsite the transferred to West Jeff hospital.
