Bird got into electrical equipment at the facility, flew into fuse, causing a power outage and subsequent SO2 release. Flare started up right away but smoked because plant was shut down and there was no steam going to it. FLARE.
Exceeded RQ. Power restored after a few hrs. Verbal/LEDQ Report only. No refinery letter.

Transfer hose ruptured, crude oil got into facility's outfall (drainage canal). RELEASE: OFF-SITE.

EXPLOSION of Belco tail gas SRU (sulfur recovery unit) "resulting in a long term loss of the scrubber" Report lists no releases/emissions, states that there were no injuries. FIRE.
BRQ. "Belco unit isolated. area barricaded. all sulfur producing units were idled in a controlled manner" Off site impact: "Some residential damage from explosion shockwave"; no environmental impact noted.

A 3/4 inch pipe nipple on the product charge line at tank 83 ruptured and released product to the diked area around the tank.
RQ. Stopped product flow and initiated cleanup. Removed product floating on water with heavy equipment. LDEQ report only. No refinery letter.
