Platform Unit Charge Heater #1 (EQT 250)
Cause: Rupture of tube in heater fire box. No 1 Platformer charge heater (EQT 0250) exceeded opacity >20% for approximately 20 minutes. The unit was turned off and cooled.

Notes: Feed and fuel into heater were removed, steamed out and water was sprayed to cool. The report states that the heater tubes will be repaired to proven recurrence. A field interview was conducted on 5/23 and photos were taken which are included in the pdf file. At the time of the inspection, a steam plum exhaust from the heater stack due to water sprayed on to the heater to cool the unit.
