Cause: LDEQ states that a ball valve was left open and released 150 barrels of mixed lube oil products.

Notes: RQ: RPR excavated the contaminated soil, and the soil was hauled to an onsite concrete pad for testing. The soil will then go to the Woolworth Landfill. The dike drain was closed, so the oil product remained in the area of tank 56. Photos were taken. LDEQ report and FIF only. No refinery letter.
